Assisted Reproduction Technology
When selecting an ART program, information is crucial. Important points for consideration include the qualifications and experience of personnel, support services available, cost, convenience, pregnancy rates and multiple pregnancy rates. Every couple wants to use the most successful ART program, but many factors contribute to the overall success of a program. Cost-effectiveness ratio is one of the main issues to be considered. Charm treatment, psychological support and individualized assessment make a good program into a very good one, where you should feel comfortable.
Who needs to undergo an ART?
It is estimated that about 15% of couples will fail to achieve conception after 12 months of unprotected intercourse. As the first treatment approach, most couples undergo controlled a controlled ovarian hyperstimulation with or without intrauterine insemination. As success rates is about 15% per cycle, ultimately, about half of these infertile couples will undergo an assisted reproductive technology procedure (ART) (in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI)).
To decide what the most adequate treatment is, some screening tests should be done:
- Hormones (FSH, LH, estradiol, TSH, Prolactin)
- Baseline Ultrasound
- Hysterosalpingography
- GC and Chlamydia culture
- Semen analysis
An ART cycle has several stages:
- Screening tests: after deciding that ART is the best option, some other screening test should be done (Type & Rh Factor, HIV 1 - 2, HBsAg, HCV core antibody, RPR (syphilis), Rubella IgG, Pap smear, CBC, Hepatogram, Glycemia (fasting), Electrocardiogram (EKG), Mammography (over 40 years old)).
- Cycle preparation: you will take some medications (i.e. contraceptive pills and/or Lupron) in order to optimize the ovarian response during the stimulation.
- Controlled ovarian hyperstimulation (COH): you will take some medications (gonadotropins) in order to stimulate your ovaries to produce several oocytes, and avoiding spontaneous ovulation.
- Oocytes retrieval: oocytes will be retrieved in operation room. This practice will be guided by ultrasound.
- IVF or ICSI: oocytes will be analyzed by the biologist who will classify them and do the fertilization.
- Embryo transfer: best two or three embryos (according to female age and medical history) will be transferred to the uterus using a very soft catheter through the cervix.
- Pregnancy test.
Egg Donation
The use of donor oocytes in assisted reproduction techniques (ART) is becoming increasingly popular. Infertile patients with poor ovarian reserve, history of poor ovarian response to stimulation, those with multiple IVF failures or advanced maternal age, are among the patients commonly treated with donor oocytes, in order to improve their chances of becoming pregnant.
In 1993, CEGYR started running an Egg Donor Program. The aim of this program is to allow getting pregnant to hundreds of women that every year would like to make true this dream but cannot use their own oocytes. Egg donation is an alternative that offers best pregnancy rates with low genetic disorders chances (lower miscarriage rate and lower chromosomal abnormalities than pregnancies with own eggs coming from older women).
The Advantages of doing it in CEGYR, Argentina
Most programs in United States use one oocyte donor for one recipient. Our program shares the donor egg cohort among multiple recipients (split cycles), allowing for a good pregnancy rate (50%) with a reduction of their waiting period and reducing costs since donor costs are shared by the various recipients, making this therapy affordable for most patients. Besides, as Argentine currency was devaluated in December 2001, costs for foreign patients are still lower.
Egg donor program of CEGYR is one of the most popular, not only in Argentina, but also in Latin America. Egg Donor screening
All our voluntary egg donors are between 21 and 34 years old and they are screened following the American Society of Reproductive Medicine (ASRM) guidelines. Their medical personal and familial history is assessed by a genetist. They have an infectious (HIV, hepatitis B and C and syphilis) and genetic (karyotype and cystic fibrosis) screening. They also are evaluated by a psychologist and a psychological test (MMPI-2) is done.
Egg recipient
It is also important to do a complete evaluation of the recipient couple in order to minimize risks and optimize chances of getting pregnant. Age limit: women up to 50 years old. Screening checklist is the following (based in ASRM guidelines):

Average time for a recipient is 2-3 months. Every couple can choose between two different plans in order to receive four or eight mature eggs. This means that no risks about an hypothetical poor donor ovarian response exist for the recipient.
Clinical outcomes (results)
Our clinical pregnancy rate is 45-50%, which is similar to most in USA and is higher than those published by the European Society of Human Reproductive and Endocrinology (ESHRE) and Latin-American registry (Registro Latinoamericano de Reproducción Asistida). It is important to emphasize that, unlike treatments using non-donor eggs, in oocyte donation, recipient age that not affects success rates, and clinical pregnancy rate is steady in women below 50 years old. It is also remarkable that egg donation is a therapeutic alternative effective and safe when it is done in a protocolized practice, following international statements based on clinical evidence.
Oocyte cyropreservation
Recent advances in the assisted reproduction technologies have now allowed us to freeze, store and later thaw and fertilize cryopreserved human oocytes. This new advance in the field of reproductive medicine provides yet another valuable tool to aid in the management of couples struggling to overcome problems with infertility. This technology allows us to offer the option of oocyte freezing and storage to women concerned about preserving their own fertility. This may be a viable option for women concerned about advancing age and its effect on their reproductive potential or for women facing premature loss of their ovaries or their ovarian function. Harvesting and cryopreservation of eggs may also be considered by women dealing with cervical, endometrial and breast cancer, as well as those facing systemic chemotherapy or other therapy thought to be a threat to reproductive potential.
When a woman is deemed to be a good candidate to produce a sufficient quantity of eggs for cryopreservation, her ovaries can be stimulated with injectable fertility medications in a similar manner to that utilized for assisted reproduction procedures. Ovarian follicle development is monitored with ultrasound measurement of ovarian follicles and estradiol levels. When the follicles are mature, HCG is administered to complete the maturation process. The eggs are harvested 36 hours thereafter, using ultrasound guided transvaginal aspiration. Mature oocytes are then cryopreserved and stored.
Then, when that woman wants to use those cryopreserved oocytes, they are thawed, and those which survive, are fertilized by doing ICSI. Success rates are still reported in a wide range from 5% to 30%. This technique has a great potential, but still should be considered as investigational.
Intracytoplasmatic Morphologically selected Sperm Injection (IMSI)
Intracytoplasmic morphologically selected sperm injection (IMSI) is a new development that may improve assisted reproduction pregnancy rates. This technology is a power light microscopy that allows embryologists to magnify sperm up to 6,000 times compared to the standard 200 to 400 times magnification associated with traditional fertilization approaches. The higher magnification can enable them to discard sperm which are less likely to produce a healthy embryo. The IMSI procedure may be a potential alternative to those couples whose semen analysis shows a severe teratozoospermia (severe alterations in sperm morphology).
Our medical and embryology staff has always been very concerned of morphology alterations. We have a very specialized team of biologists and embryologists that usually studies sperm samples in deepness. This way, we are more able to explain if there exists a systematic or a non-systematic pathology, and we assess about pregnancy chances using the own semen with a conventional ICSI.
IMSI makes it possible to discard sperm whose nuclei have an abnormal shape or contents. The optimum nucleus is smooth and symmetric, with an oval configuration. Using IMSI, the embryology team can identify any malformation or vacuole in the nuclear mass and they can choose the spermatozoa with optimal length and width.
Implantation and pregnancy by ICSI is associated with morphological nuclear normalcy of sperm. Sperm with a morphologically abnormal nucleus usually have low fertility potential, but some with certain nuclear abnormalities may still be able to produce pregnancy following ICSI. IMSI is still an investigational technology that is supposed to be at least as good as conventional ICSI, but has the potential of having better results in those cases that may benefit from it. Some papers have already published increased pregnancy rates. Nowadays, all over the world, hundreds of children have already born after doing IMSI.

Dr. Demian Glujovsky
Dr. Glujovsky, a reproductive endocrinologist and obstetrician and gynecologist, is part of the medical staff of CEGYR (Centro de Estudios en Ginecología y Reproducción / Center of Studies in Gynecology and Reproduction) in Buenos Aires, Argentina. He also serves as the Egg Donor Coordinator and the Clinical Investigation Coordinator at CEGYR. Dr. Glujovsky has completed his fellowship in Reproductive Medicine at IFER (Instituto de Ginecología y Reproducción / Insitute of Gynecology and Reproduction), Buenos Aires, Argentina, and has been approved by the Bianual Course in Reproductive Medicine at the Argentine Society of Reproductive Medicine (SAMeR).
Simultaneously, he has completed a Masters of Clinical and Health Care Effectiveness given by the University of Buenos Aires, Italian Hospital of Buenos Aires, Pan American Health Organization (PAHO), with the collaboration of teachers from the Harvard School of Public Health. There, he earned an award for exemplary qualifications. He has practiced in two recognized reproductive centers in the United States: Reproductive Medicine Associates (RMA) of New Jersey and the Center for Women's Reproductive Care at Columbia University in New York City. He has completed his university residence in Obstetrics and Gynecology at the CEMIC (Centro de Educación Médica e Investigaciones Clínicas / Center of Medical Education and Clinical Research) University. Dr Glujovsky has also earned the honor of the physician's degree at the University of Buenos Aires.
As a scientist, Dr Glujovsky works everyday on obtaining new information for improving the practice of reproductive medicine and general medicine. He is the author of numerous scientific articles and medical research. Dr Glujovsky is an investigator in the Health Technology Assessment and Economics Evaluation group in the Institute for Clinical Effectiveness and Health Policy (IECS) - and an INAHTA member (International Network of Agencies for Health Technology Assessment)-. He has been accepted into the Workshop of Systematic Reviews and Meta-analysis dictated by the Cochrane Center Coordinator in Iberoamerican Cochrane Network (Institute for Clinical Effectiveness and Health Policy) and is the main reviewer of the first Cochrane review of reproductive medicine written in Argentina.

I would like to leave a review on our recent trip to Fertility Argentina with Dr. Demian Glujovsky. Making the decision to go through with IVF after the loss of our stillborn son was very difficult. I really wanted to decrease the amount of stress there seemed to be involved with fertility treatment. Our decision to go to Argentina involved many things- the cost was much more feasible than what we had been quoted in the US. If the procedure didn't work the first time, then we would be able to afford to do it again-even including airfare/lodging. Also, being "away", or on a vacation so to speak would keep us from being focused on the procedure 24 hrs/day.
I must say our trip to Argentina was much better than we could have expected. Buenos Aires and the surrounding area's we visited were wonderful. Most people spoke English and were very helpful. And those who didn't speak it still attempted to be of assistance. It was also very easy to get around, mostly by walking but the bus system was good. (I must say the wine was great!!)
Dr. Glujovsky was kind and helpful-my husband and I were able to proceed with all of the tests etc in the US and the Dr. always responded to my emails quickly. I know first-hand that making the decision to go through with IVF is difficult and appears complicated. Its really not- I would urge anyone considering IVF to contact Fertility Argentina. Not a good experience trying to have our second child. An ectopic, a miscarriage, one transfer with no picture or evidence of an embryo, multiple failed transfers and no answers to embryo quality. Ignored and dismissed. Had to chase treatment like we were an inconvenience to them. Having to pay most times in cash always felt uncomfortable.
